Bridging the Disconnect: from Dialysis to Transplantation

December 6, 2017
Kidney Foundation of Ohio
2831 Prospect Avenue, Cleveland

Kidney transplantation is the treatment of choice for some patients with CKD and ESRD. The determination of transplant candidacy is often a complex and difficult process that requires considerable resources. During this panel presentation, participants will hear from a transplant social worker, financial coordinator, and transplant coordinator. This interactive session will provide a complete picture from when a patient expresses interest in transplantation to receiving “the call”. Participants can expect to learn how dialysis and transplant teams can work together to reach the patient’s goal of transplantation.
