Our need-based programs help patients with prescription medications and nutritional supplements, transportation costs, and emergency necessities relating to health care. Our services are tailored to empower pre-dialysis, dialysis, and transplant patients with Chronic Kidney Disease and related disorders to have a measurably improved quality of life.
Applications must be submitted by a social worker or healthcare provider.
These programs are based on the amount of funding available and is limited to the Foundation’s thirty-seven county service area in northern and eastern Ohio.

Financial assistance to pay for safe, reliable, and affordable transportation to and from dialysis or transplant appointments.

Financial assistance, available one time a year, to help pay for emergencies such as unexpected high utility bills, auto repairs, and medical supplies.
For one-time emergency assistance in the form of a gas or grocery gift card, or emergency cab ride, please submit an online form here. Request must be submitted by a social worker or healthcare professional.
Receive an emergency supply of insulin, syringes, or pen needles. Available one time only.
In an emergency, a medical alert band with information about your dialysis access site may help to keep your fistula or graft protected. Request your free identification band

Applicants must reside in one of the 37 counties in the Foundation’s service area, meet U.S. Federal Poverty Guidelines, be a U.S. citizen, and have a diagnosis of kidney disease (does not apply for Emergency Insulin). Applicant must be referred by a social worker or healthcare provider.

KFO does not provide transportation, but can provide assistance to defray the cost of transportation through our transportation reimbursement program. Patients who are approved for this program will receive a check twice a year to help with some of the expenses incurred by transportation to and from dialysis and transplant appointments. One time emergency transportation resources may also be available in the form of emergency gas cards or a cab ride, but those resources are limited and must be requested by a social worker or healthcare professional.
